What is Dengue Fever?
Dengue fever is a tropical inflammatory disease caused by mosquito bites. It has received global attention since the second world war1. The disease is common in more than 100 countries, and between 50 and 500 million people get infected each year2, with deaths reaching up to 20 Thousand lives3. It is still classified by the WHO as a neglected tropical disease.
What is the cause of Dengue Fever?
It is caused by Dengue virus, which is of the Flaviviruses genus, same as yellow fever, west nile virus and viral encephalitis4. These viruses are transmitted to the human host through bites from infected mosquitos and ticks. For Dengue virus, the mosquito known as Aedis Egypti is most typically involved5.
When the virus enters the body, it uses the white blood cells (which are the body’s own defensive mechanism) to replicate and move throughout the body carried by blood, and it can reach any organ from there6.
Another way of encountering the virus is through transfusion of infected blood and infected organ transplants, which is very common where Dengue is endemic such as Singapore7.
What are the signs and symptoms of Dengue Fever?
An incubation period of 3-14 days is expected8. The disease most commonly affects children and is more severe. The disease starts as a common cold, with fever, fatigue muscle pain in addition to vomiting and diarrhea. It often doesn’t exceed this phase and resolves on its own9.
In some cases, a severe infection occurs, and in this case the disease occurs in 3 phases:
Febrile phase: With high fever reaching up to 40o C, along with muscle pain, vomiting and diarrhea. The fever is biphasic, meaning it breaks on its own and returns within a couple of days10. A rash described as “Islands of White in a Sea of Red” occurs11, along with small bleeding spots called petechiae, and sometimes bleeding from the nose and mouth occurs4. This phase lasts 2 to 7 Days.
Critical phase: Leakage of plasma from the blood occurs, resulting in fluid accumulation in the chest and abdomen. This depletes the organs and severe organ damage may result12. Severe bleeding leading to shock can occur in a minority of cases1 , that are usually children. This phase lasts only 2 days.
Recovery phase: where the fluid seeps back to the blood stream, accompanied by itching and a rash that can cause the skin to peel off11. Fluid overload may occur, affecting the brain and causing fainting and seizures1. This phase usually lasts only 2 or 3 days, but the muscle pain could last a few weeks.

No antiviral medication has proved to be effective against Dengue. The treatment therefore depends on the symptoms, so people who don’t have febrile illness and can eat and drink and live normally, would not require hospitalization and only supportive treatment at home is sufficient.
People who have warning signs or show severe symptoms would require hospital care. Paracetamol is used to control pain and fever, and blood transfusion may be required to replace the fluids and plasma lost.
